    Consent Form
    The Participant are asked to volunteer in this study by Cookies N’ Crème Bakery, to see whether people prefer crumb cookies better than traditional cookies.

    During this research, the participant amongst 19 more voluntarily participants will be provided with one of each cookie (crumb cookie and traditional cookie) with a glass of milk, because besides who do not like their cookies without a glass of milk. The participant will take a bite of each cookie and then will be asked to vote by placing their glass of milk on the plate of the cookie the participant liked more. Therefore, the researcher will go around tallying all of the participants’ votes.

    The participant will not be guaranteed to receive any benefits, however this study will help the grand opening of the new bakery, Cookies N’ Crème whether to make more of crumb cookies or traditional cookies during the grand opening.

    The harm to the participants is any allergy the participant has that the researcher might need to know about before the study, such as dairy allergy, to see whether the participant is eligible to be part of this study.

    The participant whether or not to participate will not prejudice them from the entry of the bakery. If the participant participates than during the study he/she are consent to withdraw without penalty.

    CONSENT FORM
    You have been selected to take part in the study that tests the preference of different types of cookies. In this study, you will be ask to taste crumb cookies and traditional cookies. Then, you will state your preference of one or the other. Because the nature of the research requires consumption, please be aware of any health conditions or allergic relations you may have to dairy, sugar, wheat, gluten, eggs, peanuts, or tree nuts. If you have any of the listed allergies, please inform the researcher and the study will end now. Your part in this study is strictly confidential. Your name will not be attached to any data we collected or research reports. If you are uncomfortable with the research for any reason, you may choose to resign from the study at anytime during the research and receive monetary compensation for your participation. Although there will be no direct benefit to you for taking part in this study, the results may help researchers learn more about preferential choices of different cookies. You may request results of the experiment once the study concludes. You will be free to ask any questions or address any concerns you may have.

    Consent Form

    You have been chosen to participate in a preference test of regular cookies and crumb cookies. The purpose of this study is to see if people would like crumb cookies over traditional cookies, and if not why and how we can better improve our crumb cookies. During the study you will be given two samples, A and B, one traditional cookie and one crumb cookie, after tasting both you must write down your preference and a short reason why. The benefits of this study will be to see preferences between our cookie and others. The results will help us better our recipe based on your recommendations. Potential harm is minimal, if any. All food allergies must be reported beforehand. During the course of this experiment you may withdraw from the study at any time at no risk to you.

    Consent Form:
    The purpose of the study you are considering to participate in, is to study the cookie preferences of 20 people in the neighborhood of Just Crumb Cake Crumb’s new location in Park Slope Brooklyn. The required data is to be collected by providing each participant with two samples of cookies, in which they will then write their favorite choice on a given piece of paper. The first sample, sample A, will be a traditional cookie, while the second sample, sample B, will be one of the new crumb cookies.
    The participant will also be given a choice between milk and water. Participants will be given a choice between beverages to insure safety to those that may have allergies to lactose or dairy products. For the safety of our participants we ask, if you may have allergies consisting of dairy, wheat, eggs, peanuts, tree nuts or nut products, to please inform an experimenter before participating or signing this consent form.
    Information found in this study may be disclosed to the owners of the Just Crumb Cake Crumb owners. The findings may help the business to improve their services provided to the people in the Park Slope neighborhood. But in order to provide participants their rights to privacy, all identities will be kept confidential. No names will be connected to data or results in the study. Every participant may also ask for additional information regarding any aspect of the study.
    Participants may withdraw from the study at any time without penalty.

    The purpose of this study is cookie preferences; testing 20 participants to see if they prefer crumb cookies better than traditional cookies. The location of the collection of data will take place in downtown Blkyn, the shopping area. There will be two samples of cookies on display, crumb cookies A and traditional cookies B. Each participant has the opportunity to taste both.The results will be recorded by each participant on a piece of paper after they have made their decision of cookie preference.
    The participants will be given a consent form explaining the facts and information about the study, the purpose which is cookie preference, nothing will be done to them, they just have to taste both crumb cookies A and traditional cookies B and make a decision of the most prefered cookie. Benefits is having the opportunity to taste both cookies and making their own personal opinion on which cookie is most prefered.The only harm is allergic reaction to the cookie itself, and also that they are no forced to participate in this study, they may withdraw at anytime with no penalty.

    We also need to consider other ethical issues such as:
    1) Protecting the participants from both physical and psychological harm

    3) Confidentiality
    4) Know the purpose of the research
    5) Informed consent

    7) May contact researcher after study for more information, or request assistance with any harmful effects that might reveal themselves at a later date.

    CONSENT FORM
    You have been chosen on the random bases to participate in the research on the preference test of either crumb cookies or traditional cookies. This study involves having participants taste the cookies and state their preference, which is ultimately the goal of this research. Since the cookies will be tasted please be advised that allergic reaction to the ingredients stated on the wrapper may arise. There seems to be no potential benefit to this, the harm may be the allergic reaction of any sort. You may withdraw from this study at any time, without penalty. Your name and other information will stay confidential and will not be released to the public or anyone under any circumstances. Feel free to ask any questions, and address all your concerns. After the study has been conducted you may if you would like to obtain the results based on the data collected.

      You are invited to participate in a study designed to check the preferences of people regarding crumb cookies and traditional cookies. Please read this form and ask any question you may have before deciding whether to participate or not.
      The purpose of this study is to help better understand cookies preferences. There will be a total of 20 participants in this study.
      If you decide to participate in this study you will be asked to eat several types of cookies and then rate them according to your personal taste. You will be required to fill in a questionnaire regarding your selections.
      There are no risks or discomfort anticipated from this study. The only risks involved are in the case that the participant is allergic to any of the ingredients in the cookies and that is why we ask you to mention any sorts of allergies you might have prior to signing this consent form.
      Any information obtained regarding any of the participants will remain confidential.
      This study is completely voluntary. If, for any reason, you feel that you are no longer wish to participate in this study, you are free to leave and you will not be penalized or lose any benefits you otherwise would have been entitled to.

    A bakery is conducting research on preferences on two types of cookies (Crumb Vs. Traditional). This research study is intended to help understand society’s preferences towards cookie types. Participants will be given the two types of cookies listed above and be asked to decide which cookie they preferred more. While there will be no personal benefit to the participant, this research study will help this bakery determine which cookie type is preferred over the other. Potential harm may arise in the form of allergic reactions to the ingredients in these cookies, therefore any and all allergies are to be reported before agreeing to this study.     You have been selected to take part in our study on consumers’ taste preference in two types of cookies. Our company, Just Crumb Cake Crumbs, is conducting this research to see if consumers will prefer our crumb cake cookies instead of the traditional styled cookies sold in stores. Our test will consist of two samples labeled A and B, each representing either a crumb cake cookie or a traditional cookie. You will be asked to taste each cookie so that you can indicate your preference and the reason for your choice. During the experiment, you will be asked to be blindfolded with a cloth and to keep your hands to your sides. Each cookie sample will be hand fed to you by one of our researchers with hypoallergenic gloves on. The purpose of this procedure is to eliminate any factors that may influence a participant’s decision on taste preference. In this study, we do not want our participants to be able to identify and differentiate the two cookies before tasting it because both cookies differ in its look and texture. There is no direct benefit for the participants that choose to take part in this experiment. However, the results from this test are crucial to us because it will help our company enhance the taste of our crumb cake cookies. By molding our cookies according to consumer taste preferences and recommendations, we can satisfy our consumers with a tastier product while increasing our company’s revenue. For safety precautions, please inform our researchers if there are any food allergies or medical condition that may cause potential harm. If this is the case, your participation will end now. Your participation in this study will be held strictly confidential. All results will be used solely for our company’s, Just Crumb Cake Crumbs, research.     Consent Form:
    You have been selected to participate in a study on cookie preferences. In this study a total number of twenty participates will be taking part in this a study. These people will be tested to see if they prefer crumb cookies better over traditional cookies. The study will take place in front of the cookie store on Park Avenue in Manhattan.

    In this study you will be asked to taste cookie A and cookie B. Cookie A being crumb cookies and cookie B being the traditional cookie. The participants will also have the choice of choosing between juice, water, or milk to try with the cookie. The participants will be given a paper, where they can indicate their favorite cookie. The pieces of paper will be collected and calculated. There are no potential benefits to the participants, but is research can help the store understand their customers’ preference of cookies. For safety procedures, we ask the participants to inform the researcher of any health problems or allergies they may have to ingredients in both the cookies and the beverages. Please carefully read all the ingredients on all of the products. Some of key ingredients are: dairy, wheat, eggs, peanuts, tree nuts or nut products. If you feel that you have any health problems or allergies to these products, please notify the researcher before the beginning of the study. Please be aware that at any point of the study the participants feel like withdrawing from the study, he or she can at no penalties.

    You and 19 other participants will have the privilege in partaking in a tasty research study for a new cookie store opening in Bedford Stuyvesant Brooklyn this fall 2010. The store is called Munchies Cookies Express and the owner is curious in finding out what type of cookies New Yorkers prefer, do they like crumb cookies better than the traditional cookies? Each participant will get a sample of a crumb cookie and a traditional cookie. Then they will be asked to write down which cookie they prefer, what they like about the cookie, and ways on improving the cookie (if any). The purpose of this study is to help the owner guarantee better sales by providing the type of cookie the customer desires. There are no monetary benefits to this study but the participants will get to taste free gourmet cookies. The potential harm is minimal because you should state before hand if there are any food, dairy or nut allergies. You may withdraw from this study at anytime without penalty.     Purpose: To get 20 participants to sample crumb cookies vs. traditional cookies and report their preference.
    In this study, we will ask you to participate in an investigation that examines the preferences of different types of crumb cookies from Laura & Co. You will be asked to taste different types of cookies and state your preferences. This investigation requires your consumption of cookies. You will be given a list of ingredients that are in the cookies. You may choose not to participate in the study if you have any food allergies or simply choose not to partake in the investigation.
