    Survey Consent Form

    My name is Victoria Maksimets. I am an undergraduate student at Hunter College. I am conducting a survey as part of an assignment for Dr. Striano (Social experimental class 249.004). If you are at least 18 years old, you are eligible to participate in the survey. The survey involves answering some general questions on the topic of food in the library. I am interested in whether you can successfully sneak in the food into the library. The purpose of this study is to see whether gender has an influence on who is more likely to sneak the food in, and who is capable of following the rules. The survey takes about 5 minutes to complete. Your participation is completely voluntary, and your responses will be kept confidential. The way I will make sure of that is by locking them in my Hunter locker afterwards. You do not have to answer anything you don’t feel comfortable answering, and may withdraw at any time for any reason. By filling out the survey you are consenting to participate. This project has been reviewed by Brian Davis who is the TA for the course, and approved by the Hunter College IRB Administrator (212) 794-5504.     Consent Form
    You have been asked to participate in a study regarding the effect class size has on academic performance. The research was approved by the Hunter IRB. You can reach Paul Cascella, the co-chair of Hunter IRB, at (212)-650-3053. As a participant, you will be asked to answer a few questions about your time here at Hunter College. The survey will take approximately 5 minutes of your time. Your answers to these questions will not only help us in our research about how academic performance is effected by class size but it can also change how class size is structured in the future at Hunter. Your participation in this study will be held confidential and private and your personal information will not be released. Your data will be kept in a locked room. If at any point during the study you feel uncomfortable or are unwilling to continue to participate, you are free to stop and no longer be apart of the study and no penalty will be applied towards you. You are free to ask any additional questions you may have regarding the study. Feel free to contact the research at yxhuang@hunter.cuny.edu.

    Consent Form

    This survey is being conducted by Hunter College students for Social Experimental Research in Psychology (Psych 249) Delfino Balbuena, Natoya Bromfield, Maria Gonzalez, Stephanie Reyes.
    The purpose of this study is to find out how much knowledge individuals have on sexual harassment. At least100 participants will be asked to fill out a general survey regarding the issue of sexual harassment on the Hunter College campus. The study is completely confidential. The survey is anonymous. It will not be linked to this consent form.Your responses will be used for research purposes only and your information will not be published, distributed or shown to anyone. This study aims to gather information on how well collegiate members are aware of sexual harassment laws. This survey will not take more than 7 minutes. Once completed, the results will be posted on professorstriano.wordpress.com. If at any time you feel like withdrawing from the study, you may do so with no penalty.
